But the people in power only really care about the rich neighborhoods, and when they want to get richer, they gentrify the rest. Slowly squeeze the poorer ones to a smaller and smaller, destitute section. Use them to run drugs, or keep them addicted because gangs and crime lords have the big boys in their pay pocket.

It's... not so different, I suppose. The monarchy in my country is obsessed with conquest, and they believe that expansion is the best measure of success. They pour all their resources into the army and the war efforts, regardless of the toll it takes on the people.
It feels obvious to me where their priorities should be, but the warrior mentality is too deeply ingrained in them. They can't imagine a world where success means joining hands with our neighbours instead of conquering them.

I should say that I've never actually been committed to prison, only that my brothers enjoyed locking me in the dungeons as a prank. Not quite the same, I imagine.

There's plenty of power in grassroots movements, you don't need to be a "mayor" to effect change... Though you will need their cooperation. In most cases.
